All of the information I have given is accurate and complete to the best of my knowledge. I will not hold Christian Fellowship Center, its members, staff, or VBS volunteers liable for anything that happens while my child is in their custody and on church property. I give permission for church leaders to act on my behalf in an emergency situation in which I or my emergency contact cannot be reached and emergency action becomes necessary for my child.
